Use this form to request an item that is not available in Oakton's library database and for books NOT available through I-Share.

To apply for a digital library card/Owl ID, please fill out and submit the Library Card Request Form. Students must be registered for classes in a current or future semester to apply.

Fill out this form to give permission to someone else to check out material in your name as a "proxy patron." You are taking responsibility for any lost or damaged items checked out by this person in your name.

Use this form to report a technical issue or access issue with an Oakton library database. We will get back to you as soon as possible with a resolution. 

Faculty should use this form to request a library instruction for their asynchnrous class.

Faculty should use this form to request a library class session for their on-campus, off-campus in-person, or live-online class.

Faculty should use this form to schedule a time to bring their class to the library for a tour and so all students can obtain their O.W.L. IDs.
